Profile of need by unit
Clyfton Unit
Has a primary neurological diagnosis such as brain injury, Huntington’s disease, Parkinson’s, motor neurone disease, and spinal injuries
Presents with predominant physical health needs such as diabetes, dysphagia, epilepsy, bladder and bowel needs, and pressure area care
May have moved from a high-dependency hospital setting after an injury, or from home or another care setting as their condition has developed
Requires care for clinical interventions such as catheters and PEGs
Has limited mobility, requiring regular repositioning, skin integrity checks, and ongoing monitoring
Has communication difficulties and may be nonspeaking, requiring augmentative and alternative communication (AAC)
Requires an innovative, sensory-led approach to enhance wellbeing and quality of life
May have a respiratory condition
May require palliative or end-of-life care (for degenerative conditions)
Where there’s rehabilitation potential, individuals can transition to Bridgeway Unit within the same home to build greater independence.
For those with progressive conditions, we support person-led advance care planning, ensuring their wishes and preferences for end-of-life care are known and respected.
Bridgeway Unit
Is medically stable and requires nursing input for complex clinical needs
Has a primary neurological or mental health diagnosis and presents with predominant physical health needs
May have moved from an acute setting, such as a neuro-rehabilitation centre, where their condition has stabilised
Requires care focused on maintaining health, promoting independence, and enhancing community and social engagement
Has higher levels of mobility and communication than those on Clyfton Unit, and are more involved in self-care and daily routines
May be continuing their recovery journey with a view to transitioning to a lower acuity setting, or living well with a long-term condition in a stable and supportive environment
Bridgeway Unit has an adapted therapy kitchen, two apartments, and a private garden, offering a supportive, homely environment that supports progression and promotes independence.
The home also has a beauty therapy room and a cinema room, helping people build confidence and skills in using these services before accessing them in the community.
Whether someone is continuing their recovery journey or living well with a long-term condition, we provide personalised care that balances clinical support with a strong focus on quality of life.
Mayflower Unit
Has complex mental health needs arising from a mental health or neurological diagnosis, such as acquired brain injury, dementia, or Huntington’s disease
Displays frequent and/or unpredictable behaviours of concern, requiring structured, evidence-based behavioural support
Has experienced one or more failed placements with other care providers due to behaviours of concern
Requires high levels of observation upon admission, often reducing over time as wellbeing stabilises
May move to other units in the home when behaviour stabilises
Many people move to Mayflower following the breakdown of previous placements - often because other providers don’t have the expertise, skills, or staffing levels to deliver this level of care safely and consistently.
For those with degenerative conditions, we support transitions to other units in the home as clinical needs increase, ensuring continuity of care in a familiar environment.
Pilgrim Unit
Has complex mental health needs arising from a mental health or neurological diagnosis, but doesn’t require high-level interventions or constant observation
Displays behaviours of concern at a lower level of risk to those on Mayflower Unit
Is vulnerable to or unsettled by more intense behaviours, now benefiting from a calmer, social environment with consistent routines and lower-level support
Is stepping down from more intensive behavioural support services, including Mayflower Unit
Requires proactive support, routine, and meaningful occupation in a social setting
Requires a balance of safety, structure, and community connection, where people enjoy being part of daily life both in the home and beyond
