The people we support

It can be a struggle to find a care home for a loved one with complex care needs. At Exemplar Health Care, we’re here to help. 

We care for people who have a primary dementia, mental health, neuro-disability, or physical disability diagnosis, as well as one or more secondary conditions or disability.

Some of our homes are also registered to support adults living with severe or profound and multiple learning disabilities.

No matter your loved one’s needs, we’ll show you how to find a care home where their wellbeing comes first.

The right care, the first time round

Not all types of care homes have the right facilities and expertise to provide complex care. Often, this means the placement fails, and you must restart the process of choosing a care home.

We know how distressing this is for you and your loved one.

Our colleagues have the experience, knowledge, skills, and values to meet complex care and health needs.

At Exemplar Health Care, over 50% of the people we support come to us following a failed placement with another care provider.

Often, they go on to thrive - feeling encouraged to enjoy meaningful lives.

Care from a highly trained team

With our highly trained colleagues, your loved one will always be the saftest hands. 

Every home has a its own Home Manager and Clinical Nurse Manager, as well as a team of Unit Managers, Registered Nurses, Health Care Assistants, Activities Coordinators, and an in-house support team.

Many also have in-house an Physiotherapist, Occupational Therapist, and/or Therapy Assistant.

Exemplar Health Care clinical experts, including Behaviour Support Practitioners, are on hand to support your loved one, if needed.

Our staffing levels are based on individual need, and there’s always a Registered Nurse on duty. Not only does that give your loved one constant access to nursing care, but they also have more opportunities to build strong relationships with those who support them.

Welcoming and supportive environments

Our homes are designed into small, often 10-bed, communities. This means we can provide more highly focussed care, and individuals have chance to build stronger bonds with those they live with, if they choose to.

An Exemplar Health Care setting should feel like home. Each bedroom has an en-suite for privacy, and we encourage your loved one to bring personal items with them to decorate their room, where it’s safe to do so.

Our homes also have a range of communal rooms, designed to build communities and welcome families.

These may include an activities hub, beauty salon/hairdressers, multi-faith room, sensory bathroom, sensory room, therapy room, visitors’ suite, and large garden.

  • Bright and spacious accommodation, either purpose-built or adapted to meet people’s needs

  • Fully furnished en-suite bedrooms, which can be decorated to individual taste

  • Comfortable lounges, dining areas, and activities hubs for socialising

  • Regular trips and outings, as well as events and parties in the home

  • Group and one-to-one activities, both inside and outside the home

  • Quiet areas to enjoy time with friends and family - some of our homes also have guest rooms for overnight stays

  • An open-door policy, so friends and family can visit whenever they like

  • Trained colleagues, who have the right knowledge, skills, and values to care for your loved one
