Planning for what matters most
Every individual we support has a voice to express their goals, preferences, and needs, through collaborative and person-centred care planning.
We take a holistic, whole-person approach, which meets not only clinical but also emotional, mental, physical, social, sexual, and spiritual wellbeing needs.
Each person we support has a My Exemplar Care Profile. This is hosted in Nourish Better Care, a widely used digital care management platform, and includes robust care planning for over 16 care domains.

A collaborative approach
Wherever possible, people help create their own care plans using proven tools and approaches, such as:
good day/bad day
one-page profiles
Planning Alternative Tomorrows with Hope (PATH).
These tools not only empower people to share what matters most to them but also enable us to tailor support that makes a meaningful difference.
Whether identifying goals, understanding personal routines, or planning milestones, the care profile is a live document that truly represents the individual.

Reducing risks in non-restrictive ways
At the heart of our care planning is a flexible, person-centred approach to risk management.
Many of the people we support live with complex needs that require careful, clinically informed decisions to balance safety with independence.
We work with each person to understand their needs, preferences, and goals, putting personalised plans in place that reduce risks without unnecessary restrictions.
Through ongoing assessments and collaborative planning with families and professionals, we empower people to make informed choices while staying safe.
This flexible, clinically informed approach helps people live full, meaningful lives, with support tailored to their individual circumstances.
Focus on empowerment, not control
People have a right to make choices, even if those choices involve risks. We help them take positive, well-managed risks to promote independence.
A collaborative approach
Done from admission, we explore the specific factors contributing to the risks people face. We work closely with them to implement strategies to support their independence and safety.
Enhancing lives in meaningful ways
Risk management shouldn’t be a hinderance. We make sure people enjoy their lives in ways that matter to them as individuals, minimising harm as much as possible.
Continuity of care for quicker responses
Every unit is staffed by a consistent team. Colleagues understand people’s needs so can respond quickly when necessary, reducing the risk of harm.
