Every individual we support has a voice to express their goals, preferences, and needs through collaborative and person-centred care planning - which forms their My Exemplar Care Profile, hosted in the Nourish Better Care digital care planning system.
Through robust care planning for over 16 care domain, we ensure a holistic, whole-person approach, which meets not only clinical but also emotional, mental, physical, social, sexual, and spiritual wellbeing needs.

Our partnership with Nourish, an NHS Assured Supplier of social care technology, enables us to record, plan, and coordinate care in real-time.
Using secure, accurate digital records supports better decision-making and more effective teamwork.
With portable hand-held devices, our teams can capture and analyse data instantly, allowing for more responsive, tailored care. Information flows smoothly across shifts and teams, with care notes immediately accessible to colleagues whenever they need them.
The system also supports greater involvement from people in reviewing and shaping their care plans, promoting independence and ensuring their wishes and preferences remain at the heart of their care.
