Who we support

We provide 24-hour nursing care for adults with complex, co-occurring needs, helping people stay well, maintain their independence, and live a meaningful life.

Someone might come to live with us when they are:

  • needing increased nursing support as their needs change or progress
  • stepping down from hospital or a secure setting
  • moving from a care provider that can no longer safely meet their needs
  • looking for a new home following a previous placement breakdown.

Our two care communities have distinct profiles of need, helping us match each person with the right care, environment, and community to support a successful, sustainable placement.

Typically supports adults with:

  • a primary neurological diagnosis in the advanced stage, such as brain injury, Huntington’s disease, motor neurone disease, or spinal cord injury
  • predominant physical health needs that require a high level of nursing input, observation, and monitoring
  • co-occurring health needs such as diabetes, dysphagia, and epilepsy
  • clinical interventions such as catheters and enteral feeding
  • dysphagia, requiring modified diets, supervised mealtimes, and aspiration risk management
  • limited mobility, requiring regular repositioning, skin integrity checks, and ongoing monitoring
  • co-occurring mental health needs with associated behaviours of concern.

Care focuses on maintaining function, comfort, meaningful involvement, and independence – and adapts through increasing frailty, palliative care, and end-of-life care.

What makes our care different

Our approach brings together nursing, clinical, and behavioural expertise, staffing shaped around individual needs, and an environment where people can live well.

24-hour nursing care

Registered Nurses are on site 24 hours a day, providing responsive care, clinical oversight, and early recognition of change.

Needs-led staffing

Staffing and observation levels are shaped around individual needs, risks, and complexity, adapting as a person’s needs change.

Multidisciplinary approach

Nursing, medical, therapeutic, and behavioural expertise works together to provide coordinated care around each person’s needs.

Complex nursing and clinical needs are supported alongside neurological, mental health, and behavioural needs.
