Profile of need
Has complex co-occurring mental and physical health needs typically arising from a primary neurological diagnosis, including brain injury, dementia, Huntington’s disease, or motor neurone disease
May have a secondary mental health diagnosis with associated behaviours of concern
Requires a high level of clinical nursing input and observation to monitor for deterioration or relapse
Presents with increased clinical and safety risks, including infection, malnutrition, falls, or risks related to personal safety
May have a respiratory condition, including asthma, chronic obstructive pulmonary disease (COPD), or pulmonary hypertension
Requires advanced nursing and clinical interventions, including catheter and stoma care, central venous access management, enteral feeding, tracheostomy care, non-invasive ventilation (CPAP/BiPAP), or mechanical ventilation
May have cognitive impairment affecting memory, judgement, and awareness of risk
May require nursing support for wound care, including assessment, dressing changes, monitoring for infection, and adherence to clinical care plans
May experience progressive health decline associated with increasing frailty
May require continuing care through to end of life

Thames House is a short drive from the M62 and other major roads including the A6193.
If you're travelling west on the M62, exit at junction 2, towards Milnrow/Shaw/A640.
If you're travelling east on the M62, exit at junction 20 towards the A627(M).

There are limited parking spaces in the home’s car park. Street parking is available along Thames Street.
