Our step-in and step-out OneCare services for adults with complex needs

Terry lives at the OneCare service at our Neville Court home in Barnsley. Since moving into the service, he’s built his independent living skills and now manages his own everyday living.

Terry has lived in Sheffield since he was born and experienced a challenging childhood, often sleeping rough whilst growing up.

In 2019, he was referred to Neville Court by his Social Worker, who was concerned over his dangerous hoarding and ability to take care of his own basic care needs - his house didn’t have electric, gas or running water.

Since moving into the OneCare service at Neville Court, Terry has progressed significantly and is now thriving in his abilities to take care of his own personal care and hygiene, as well as everyday living skills such as cooking and budgeting.

Terry takes pride in his appearance and is now enjoying freshly cooked meals daily and a clean home environment.

About our OneCare services

Our OneCare services offer a step-in and step-out service for adults living with complex and high acuity needs.

They’re a mix of single and shared occupancy houses and flats that are next door to our care homes.

This means that people can access specialist nursing care and clinical intervention from a known Care Team, in a more independent living environment.

Step-in and step-out services for adults with complex needs

Our OneCare services are registered with the Care Quality Commission (CQC) for nursing care, and provide a range of flexible support packages that are built around people’s needs and goals.

• Step-out service: for people who want to develop their independent living skills as part of their journey from residential nursing care to community-based living/lower acuity care services.
• Step-in service: for people who require complex nursing care but who aren’t quite ready to move into a nursing home, such as people who are in the early stages of Huntington’s disease.
• Single occupancy: some of our OneCare services may also be a great fit for people living with complex care needs who aren’t suitable for group living.

Why choose OneCare?

Our OneCare services support people living complex and high acuity needs, unlike many supported living services.

People living in OneCare can access specialist nurse-led 1:1 clinical support from a team of Nurses, trained Support Workers and Life Skills Facilitators.

They also have access to Exemplar Health Care’s in-house team of Behaviour Support Specialists, Physiotherapists, Occupational Therapists and our Consultant Psychiatrist. 

OneCare offers care, nursing and accommodation costs in one weekly fee, which enables people to progress on their journey at their own pace, without the need to apply for other benefits or set up utilities, and allows a straightforward reassessment and transfer between our care homes and OneCare services.

If needed, people can also access short-term support from the nearby home, to avoid a re-admission to hospital.
