We provide 24-hour nursing care for adults with complex, co-occurring needs, helping people stay well, maintain their independence, and live a meaningful life.

Someone might come to live with us when they are:

  • needing increased nursing support as their needs change or progress
  • stepping down from hospital or a secure setting
  • moving from a care provider that can no longer safely meet their needs
  • looking for a new home following a previous placement breakdown.

Our two care communities have distinct profiles of need, helping us match each person with the right care, environment, and community to support a successful, sustainable placement.

Typically supports working-age adults with:

  • a primary neurological diagnosis, such as brain injury, motor neurone disease, or stroke
  • predominant mental health and behavioural needs
  • co-occurring physical health needs, including diabetes and dysphagia
  • a forensic background, where risk-related behaviours have reduced or ceased over time.

Care focuses on stability, emotional wellbeing, meaningful engagement, daily living skills, and greater independence, with nursing support for co-occurring physical health needs.

Typically supports working-age adults with:

  • a primary neurological diagnosis, such as brain injury, motor neurone disease, or stroke
  • predominant physical health needs that require a high level of nursing input, observation, and monitoring
  • co-occurring health needs such as contractures, diabetes, dysphagia, epilepsy, and respiratory conditions
  • clinical interventions such as catheters and enteral feeding
  • complex respiratory needs requiring non-invasive ventilation
  • limited mobility, requiring regular repositioning, skin integrity checks, and ongoing monitoring.

Care focuses on maintaining function, comfort, meaningful involvement, and independence.

For people with progressive conditions, care can adapt through increasing frailty, palliative care, and end-of-life care.
