Our holistic approach to care is trusted by countless families and referring professionals across the country. At Exemplar Health Care, we’re proud to be one of the country’s leading providers of specialist nursing care, supporting adults living with a range of complex care needs.

Our aim is to make every day better for the people we support, and we do this by putting our people at the heart of everything that we do.

Personalised approach to care

Our approach is personalised to the individuals that we support – we work with them, their loved ones and professionals to design a care plan that’s tailored to their goals.

This ensures that every person living in our care homes is enabled to meet their physical, emotional, vocational, educational, spiritual, safety and social needs.

“Before Arlene came to Fairburn Mews she was emotionally distressed - but Arlene is in a much better place now. The staff at Fairburn Mews are very supportive to Arlene and myself, and have gone above their duty - they are amazing.”

Holistic approach to care

Each of our homes has a skilled in-house team of Nurses, Health Care Assistants, Therapists and Life Skills Facilitators, as well as access to Exemplar Health Care’s Behavioural Support Team.

This enables people living with complex needs to access holistic care in a community-based home that’s closer to their friends and family.

“I have a young lady with severe Huntington's disease, who was being cared for by her family and a package of care at her home. As her disease progressed, it became a crisis situation; not only for her, but for her very much devoted husband. From my very first phone call to [the Manager], to this lady's now current situation as a permanent resident of Church Walk, the holistic care of this lady and her family, has been nothing but excellent.”

High staffing levels

Our high staffing levels enable us to work at people’s own pace and build trust, which leads to improved person-centred and longer-term outcomes.

Most of our homes are designed into 10-bed units, each of which are led by a Registered Nurse or experienced Team Leader, depending on the unit’s specialism.

Our Nurse to Health Care Assistant ratio is one of the best in the country – with an average of one Nurse to every six Health Care Assistants.

We invest in our workforce through an extensive learning and development programme, to ensure that our colleagues are confident and competent to deliver the highest standards of care.

“I’ve loved living at Copperfields – the staff and Manager are great. They listen to me and what I want to achieve, and the home has an ethos of progressing, learning and moving forward – which is what I want to do in my life.”

The right care, the first time round

Over 60% of our residents come to our homes following a failed placement. We adapt our approach to each individual which brings an end to this cycle.

“I am a Social Worker who has recently placed a client at Tyne Grange. She is a complex lady with a number of failed placements in her past. I have been very impressed with Tyne Grange - from the initial referral they demonstrated genuine curiosity regards the client and were interested in how to create a care plan to keep her safe and meet her needs. Since she moved, she has continued with her recovery and is happy and settled within the unit. Communication with the staff has been excellent and I recommend Tyne Grange to my colleagues.”

Service user engagement

We ensure that people are included in all decisions about their care and support, either directly or through family or advocates.

Service users can also get involved in decisions that impact the running of our homes and the wider company.

Each of our care homes has a Service User Ambassador who represents their home at our monthly Service User Council meetings.

We also encourage people to get involved in company-wide projects such as updating our service user holiday policy, and play an active role in their home, such as being a buddy, getting involved in recruitment and taking part in training sessions.

“There were consistently high levels of engagement with people using the service, families and other professionals … and a strong commitment to ensure the service was inclusive and that people had the opportunity to live meaningful lives.”
