Rehabilitation nursing and support

Following hospital treatment, or recovery from an operation, many individuals are not ready to return home straight away.

Rehabilitation needs differ between individuals. At Exemplar, our specialist care teams determine individual needs and rehabilitation required to encourage people to regain their independence, confidence and the assurance they need to return home.

Specialist nurses within our care homes will carry out an assessment with the individual in need of support, their families and professionals to create a personal care plan. This plan is uniquely tailored to the individual, and will set out short and long term goals to encourage self-development.

Exemplar’s homes work with physiotherapists, speech and language therapists, multi-disciplinary teams and more – who all work together with service users, and individually to ensure goals are met and independence is increased.

Following the completion of their rehabilitation plan, individuals will have been supported to achieve their goals – whether that’s to return home or to extended care pathways such as Exemplar’s OneCare services. Exemplar’s OneCare services are houses and flats located onsite, or very close to our homes. OneCare services are extended pathways of care, allowing for independent living – still with the access to care as and when required.
