Through our robust assessment process, which includes a community fit assessment, we ensure that we're right for the individual and that the individual is right for us.

This approach ensures that people get the right support, at the right time, the first time round - which reduces re-admissions to acute settings, facilitates faster returns to lower-cost community living and brings an end to the cycle of failed placements.

The right care, the first time round

60% of our residents come to our homes following a failed placement.

Our holistic approach to care brings an end to this cycle and is trusted by more than 100 ICBs and local authorities who refer to our homes.

Our approach to care

[The Home Manager] is compassionate and probably the most person-centred and creative Nursing Manager I have had the privilege to work with. She cares deeply for our patients and really strives to know them at the most sensitive time of their lives. This compassion not only extends to the patients but also their families.

I’m enormously grateful for the support of the whole Brook View Team who were instrumental in getting Steve to a point where going home become a reality. He's now using a stick around the house instead of a frame. He’s attending a day center two days a week and has just started sessions with a private Physiotherapist.
