Our approach to care is personalised to the individuals that we support. We work with them, their loved ones and professionals to design a care plan that’s tailored to their goals.

This approach ensures that people get the right care, at the right time, the first time round. This reduces re-admissions to acute settings, facilitates faster returns to lower-cost community living and brings an end to the cycle of failed placements.

Our commitment to quality is reflected in the ratings that we receive from Care Quality Commission and endorsements from health and social care professionals, charities and support organisations.

Community-based care for adults with complex needs

Exemplar Health Care is a leading provider of specialist nursing care for adults living with a range of complex and high acuity needs.

We care for some of the most complex and acute individuals, who would often be in a hospital setting if not placed with us, enabling them to stay in the community and reducing hospital and secure facility admission.

Many of the people we support have been deemed to have behaviours that are considered ‘too complex’ for mainstream nursing and care services. All of our care and nursing colleagues complete our Exemplar Positive Behaviour Support (EPBS) training that’s been certificated by Bild ACT, and are supported by our central team of Behaviour Support Specialists through person-centred interventions and regular forums. 

Holistic care from trained in-house teams

Our high staffing levels of Registered Nurses and trained Health Care Assistants are determined by each person’s individual needs. This enables us to work at their own pace and build trust, provide support with all aspects of their life and ensures the safety and welfare of everyone living in the home, which leads to improved person-centred and longer-term outcomes.

Each of our homes has an experienced Nurse-led management team and a trained in-house team of Registered Nurses, Health Care Assistants and Life Skills/Activities Coordinators, as well as access to our central clinical experts, Consultant Psychiatrist and Behaviour Support Specialists.

Some of our homes also have in-house Therapy Teams, including Physiotherapists and Occupational Therapists.

This enables people to access holistic care and a range of services in a community-based home that’s closer to their friends and family.

At Exemplar Health Care, we have a dedicated in-house Quality and Compliance Team to ensure quality and drive continuous improvement.

Our approach is personalised to the individuals that we support – we work with them, their loved ones and professionals to design a care plan that’s tailored to their needs and goals.

We encourage people to continue to do the things that they enjoy, as well as develop new hobbies and access education and volunteering if they wish. And we adjust our approach as a person’s condition develops and changes.

Finding the right nursing care for adults living with complex and high acuity needs can be difficult. Not all services have the right facilities and expertise to meet people’s complex needs, particularly those who display behaviours of concern, which can often lead to placements failing. This causes significant distress for individuals and their family members, as well as increasing the cost to the local health and social care system.

At Exemplar Health Care, we have the right experience, skills and staffing levels to provide the specialist care required to support those living with complex needs. Around 60% of our service users come to us following a failed placement.

Our high staffing levels and holistic approach to care ensure that people get the right care, at the right time, the first time round. This avoids the distress experienced by people and their families when their placement breaks down, as well as reducing re-admission to acute settings and decreasing the cost of care to commissioners.

Our workforce is our biggest asset, which is why we invest in them. We put our values at the heart of our recruitment and retention, and we have a strong learning and development culture from induction to CPD, as well as a robust health and wellbeing programme.

We’re one of the largest private providers of the Trainee Nursing Associate programme, offering training and career development for Health Care Assistants who want to develop their clinical skills.

The right care, the first time round: Annie's story

Annie moved into our Copperfields care home in Leeds following a failed placement at another care provider. She now gets the care she needs, from the trained team at Copperfields, which has supported her to become more independent. She lives a fulfilling life at Copperfields and enjoys taking part in a range of activities, including being the Service User Ambassador at the home.
