Welcome to Preston
Work to build a new specialist care home has begun on the site of the former St Mary’s Church, Church Avenue, Preston.
When complete, Exemplar Health Care’s newest home will provide care for up to 30 younger adults with a range of complex needs including neuro-disabilities, learning disabilities and autism, mental ill-health, dementia, brain injury and stroke.
The new specialist service will support people to achieve their rehabilitation goals and become as independent as possible. A multi-disciplinary team including specialist nurses, support workers and therapists will work closely with consultants, GPs, allied health professionals and mental health teams across the North West.

The home’s high-quality care and clinical teams will work with individuals to achieve personal health care goals, become more independent and help make every day better.
A multi-disciplinary team including specialist nurses, support workers and therapists, will work closely with consultants, GPs, health professionals and mental health teams across Yorkshire. The services provided at the Leeds home will form part of a pathway of care options for those individuals who are looking to move into more independent and community based living.
