|OneCare service||Goals & outcomes|
Lonnen Grove’s OneCare service, a two bedroomed terraced house, is easily accessible from the main home - just a short walk across the road.
Each bedroom has an ensuite bathroom and the shared lounge, kitchen and dining room allow service users to interact with one another, and their guests, whilst also developing their independence, in a home from home environment.
The house can be adapted to suit the individual needs of service users, from decorating to their personal tastes to ensuring their home is well-suited to their physical requirements.
Lonnen Grove encourages service users to work towards achieving their personal goals, and further develop their independence and life skills. Service users can move into the OneCare service when they are ready – whilst still having the support of the main home as and when they require.
Supported by specialist nurses and life-skills teams, service users can further progress their journey to independence before returning to life within the community.
Steven came to Lonnen Grove with severe learning disabilities, which caused him to become extremely anxious and agitated. Over time, supported by the mental health and general nursing team, Steven was able to stabilise his mental health and make progress towards his personal rehabilitation goals.
As he progressed further and his confidence grew, he was supported by the Lonnen Grove team to develop skills of everyday living including cooking, cleaning and doing his own laundry. Steven is now looking forward to moving into the OneCare service where he will be able to live independently with the support of the Lonnen Grove team close by should he require it.
"The OneCare service has been a long time coming – it will make such a difference to the lives of our service users; encouraging them to progress with their personal outcomes and life within the community.” Kelly Allen, Home Manager
