Care that brings the right expertise together

Multidisciplinary approach

Complex and co-occurring needs require care that looks at the whole person.

Our teams bring together the right expertise to provide joined-up care around each person’s needs and goals.

One team, working around the person

A change in one area of someone’s health or wellbeing can affect many others.

Our nurse-led teams work alongside therapists, clinical professionals, and external healthcare partners to understand how someone’s needs interact.

By sharing knowledge and working towards the same priorities, teams can recognise changes, coordinate care, and adapt support around the person.

This helps people stay well, maintain or develop their independence, and live a meaningful life.

Our multidisciplinary approach

Whole-person understanding

Looking beyond individual diagnoses to understand how someone’s needs interact.

Shared expertise

Bringing together the right professional knowledge for each person.

Coordinated care

Agreeing priorities so everyone works towards the same goals.

Early recognition of change

Noticing changes in health, behaviour, or abilities and responding early.

Consistent support

Putting agreed approaches into practice as part of everyday care.

Ongoing review

Adapting care as someone’s needs, abilities, and goals change.

The right team around each person

There isn’t one standard multidisciplinary team for complex care.

The professionals involved depend on each person’s needs, circumstances, and goals, and may change as their needs change.

Our Registered Nurses and care teams work alongside Exemplar Health Care’s therapists and clinical professionals, as well as GPs, NHS teams, and other external healthcare partners.

The right care from the start

Supporting decisions around complex referrals to understand each person’s needs and ensure the right care, staffing, and clinical support are in place.

Greater continuity of care

Adapting care as someone’s needs change, helping maintain stability and reduce the risk of unnecessary moves or placement breakdown.

Earlier, coordinated support

Bringing together the right clinical and professional expertise when someone’s health, behaviour, abilities, or support needs change.

Confident, skilled teams

Building knowledge and confidence within our home teams, with access to additional expertise when people’s needs become more complex.

Clinical expertise that supports our homes

Our home teams can access clinical and professional expertise from across Exemplar Health Care.

These teams provide advice, assessment, guidance, and training, working alongside colleagues to shape and strengthen each person’s care.
