End of Life Care/Palliative Care

We all know that this is a difficult subject, and talking about it can be hard, but you can be rest assured that we will provide comfort, dignity and support in your time of need.  Family time is incredibly important during these days and fulfilling the individual’s wishes’ are of paramount importance.  All these factors have helped shape our palliative and end of life care.

When an individual is no longer able to stay at home or in hospital we ensure that we make the transition to our ongoing care as comfortable and dignified as possible.

All our staff are trained to provide only the best high level of care at this difficult time, and we follow the Gold Standard Framework of Care for the End of Life Pathway.  Staff will make admission to the home as seamless as possible with the support of general practitioners, local palliative teams, MacMillan Nurses and many more.
