Working with funders and individuals in care settings, we have developed a range of OneCare services – specially adapted individual houses and flats for people who do not require the wraparound care of a home, but still need support. OneCare services are located either on site, or very close to our care homes – sometimes down the garden path. OneCare services allow any type of care or nursing to be extended from the main home, whilst allowing individuals to live independently or with support, depending on their unique needs. OneCare services offer three distinct integrated care solutions: OneCare: Independence An extended pathway of care for individuals to maximise their independence and reach their full potential. OneCare: Intermediate A pathway of care which offers support to maintain independence or provides a rehabilitation pathway. OneCare: Individual A sole-occupancy environment personalised to suit the unique needs of individual service users.
